Responsibility Grid

Students

Office of Disbility Services

Admission

Upon admission or prior to admission contact the Office of Disability Services and provide documentation. Be available to explain the admission and documentation process and services. Begin a file for student.

Assessment

Once documentation is on file discuss needed and reasonable accommodations and agree to accommodations for assessment. Review documentation and discuss needed and reasonable accommodations for the assessment.

Registration

Meet with advisor to plan appropriate program of study. Assistance with registration and planning program of study taking into consideration impact of disability on study.

Instructional Accomodation

Academic accommodations need to be requested and discussed with the Office of Disability Services and instructor. General agreement must reached on need and reasonableness of accommodations. Discuss and determine reasonable and needed accommodations. Communicate with instructors regarding needed accommodations and to confirm the student�s eligibility for accommodations.

Mid Semester Reports

Agrees to have mid semester progress reports from faculty sent to the Office of Disability Services. Initiates the student�s agreement for the mid semester reports. Students will receive the faculty feedback during advisement or prior if there appears to be a problem.

Advocacy

Learn to present their strengths and needs to the �public� as well as successful solutions, Office will help. Office to assist students as they learn to negotiate the educational systems and present their needs in appropriate ways.

Confidentiality

All information given to the Office of Disability Services will only be shared with faculty that student identifies as needing specific information and administration. Holding all information related to disability confidential within the Office of Disability Services. Will only release information with signed permission of the student.

Tutoring

Requests peer tutoring that is available to all students. Refer student to tutoring services.

Release of Information

Student identifies who they wish to have information about their college performance (if anyone). Staff keeps signed information release in student�s folder.
