Academic Accommodations Request Process

Accommodations and services are not automatically provided and it is the responsibility of students to notify the Office of Disability Services of the need for accommodations and to provide appropriate written documentation of the disability. 

   
  • Prior to receiving academic accommodations and services, students are required to provide the Office of Disability Services (ODS) with recent written documentation (i.e., within the last 3 years) of their disability from a licensed or otherwise properly credentialed professional who has undergone appropriate and comprehensive training. Documentation must be written on official stationary or letterhead; documentation written on a prescription pad is not acceptable. 
 
  • Subsequent to providing appropriate documentation, each semester students must formally request accommodations via completing and returning an Academic Accommodation Request Form (ARF) to the ODS. Students are encouraged to complete the form and a current class schedule within the first two weeks of the semester and each student is strongly encouraged to meet with the Office of Disability Services staff to discuss the appropriate academic adjustments and auxiliary learning aides.  
 
  • Once the ARF has been received, if approved, within 48 hours the ODS will prepare individual Letters of Accommodations for each course. The letters are distributed via e-mail and interdepartmental mail to faculty members.
 
  • Students are expected pick-up copies of their Letters of Accommodation from the ODS.
 
  • Please Note: Approved accommodations are based upon formal documentation of a disability. The ODS reserves the right to request additional documentation if the documentation provided is deemed inadequate. Disability information and documentation is held confidential and secured by the Office of Disability Services.  It is only released with the specific written permission of the student.  While appropriate college officials may have access to student records, information is released on a need-to-know basis only.
