Use this form if you want to have equipment delivered to you on the same day or days of the week throughout the semester.  For instance, if every Tuesday and Thursday at 10:00 a.m. you wanted a TV and VCR, you would choose the Recurring Requestform.

Use this form if you want to have equipment delivered to you on a specific date.  If you wish the same equipment delivered on two or more dates, fill out a form for the first date and send, then go back and change the date requested field to the second date.  For instance, if you want a microphone delivered at 1:00 pm on 6th and again on 22nd, use the Specific Date Request form.
